Thank you for choosing Life in Balance for your care. A missed appointment without advanced notice does not allow for another patient to benefit from Dr. Gruber’s time. Because of this, we require a $100 deposit when scheduling new patient appointments. If you do not cancel your appointment, this deposit will be applied to your co-pay, deductible, or treatment costs at your visit.
How It Works:
- Deposit Payment: A $100 deposit is required at the time of booking. We will process this payment over the phone or through our secure online payment system.
- Application of Deposit: At your appointment, the deposit will be applied to any out-of-pocket costs.
- Cancellation Policy: If you need to reschedule or cancel, please notify us at least two (2) business days in advance. Deposits for late cancellations or no-shows are non-refundable.
